Privacy policy
How this website handles appointment inquiries. Last updated November 7, 2026.
Who operates this site
This policy covers Lisa's Medical Massage Studio, 1101 N Ward St, Tampa, FL 33607, USA. Privacy questions use the form on this page, or [email protected] and +1 (813) 252-4901.
Information the appointment form collects
If you submit the appointment form, this website stores your name, email address, selected service, message, and the time of the submission. If you type them, it also stores a telephone number, a preferred date, and a preferred time of day. It stores a hash of the network address for rate limiting. The raw network address is not stored in the inquiry record. A private server log notes that an inquiry was saved. It does not store your name, email, telephone number, or message. The form does not send the inquiry by email and does not place a call.
Do not send medical history, diagnoses, treatment records, or other health details. The form does not ask for them. If you type them into the message anyway, they are stored with the rest of the message. This website is not a medical record system, and it does not claim to be a HIPAA-covered service.
Loading the form sets one session cookie, described in the cookie policy. The site does not use analytics, advertising pixels, or other optional cookies.
Why it is used
The record is kept so the studio can review what was saved, block automated abuse, and delete it on the schedule below. Saving a record does not send it by email, place a call, or create an appointment. It is not sold and it is not used for cross-context advertising. No advertising or analytics service receives the form contents.
Where it goes
The record is stored in a private database on this server. That database is not published as a web page. The form does not email the inquiry. A saved inquiry is not an appointment.
How long it is kept
Inquiry records are deleted 180 days after they are saved. The website runs that deletion at least once a day when the site is used, including when a form is opened. It does not wait for a new inquiry. The same check removes privacy confirmation codes older than 48 hours, security tokens older than four hours, and rate-limit counters older than about a day. The same deletion can also be run by a server command if the host schedules one.
Access, correction, and deletion
You can ask to see the inquiries stored under your email address, or ask for those rows to be deleted. The form on this page creates a confirmation code and shows it here. It does not email the code. Nothing is copied or deleted until you submit that code on this page. The code expires after 48 hours.
After you confirm an access request, the stored inquiries are shown on this page. They are not emailed. After you confirm a deletion request, the matching rows are deleted and the page states how many were removed.
The code is shown to the person who submits the form. It is not sent to the inbox, so anyone who can submit this form for an email address can see or delete the inquiries stored for it. Use the email address that is on your own inquiry.
Correction of a saved inquiry is handled by deleting the old row after a confirmed request and submitting a new inquiry. There is no separate edit screen.
Depending on the state where you live, consumer privacy laws may give you rights to access, delete, or appeal a refusal. This site does not sell personal information and does not share it for cross-context behavioral advertising. The form is not for children, and you should not submit information about a child.
Privacy request
Choose access or deletion, and use the email address that is on the inquiry. Sending this form shows a confirmation code. It does not itself delete or display the inquiry.
Confirm a code
The code appears in the message above and in the field below. It is not emailed. It expires after 48 hours.
Changes
If this policy changes, the date at the top of the page changes with it.
